La Complexité De La Relation Médecin/malade : La Rencontre Patient âgé / Médecin

Résumé L’objectif de notre travail est de comprendre et d’analyser, du dedans, la construction de la rencontre patiente âgé/ médecin au cours de la maladie chronique. Cette dimension primordiale dans le travail de soin est souvent occultée dans les multiples études ayant comme objet la relation thérapeutique. Or elle représente un élément incontournable dans la définition de la nature de la relation entre les protagonistes. Il est indispensable de produire une théorie expliquant comment le profane rencontre le médecin (Stimpson G, Webb B, 1975). Cette situation de soin complexe, est envisagée comme une succession d’interactions permettant au patient âgé d’évaluer la qualité des prestations du praticien consulté. La complexité de cette situation de soin met en exergue différentes logiques, manœuvres et pratiques des protagonistes, donnant sens aux rapports médecin-patient. Il est donc, nécessaire d’analyser de l’intérieur le contexte dans lequel le patient rencontre le médecin. Abstract : The objective of our work is to understand and analyze, from within, the construction of the elderly patient/doctor encounter during chronic illness. This essential dimension in care work is often overlooked in the many studies having as their object the therapeutic relationship. Yet it represents an essential element in the definition of the nature of the relationship between the protagonists. It is essential to produce a theory explaining how the layman meets the doctor (Stimpson G, Webb B, 1975). This complex care situation is considered as a succession of interactions allowing the elderly patient to assess the quality of the services of the practitioner consulted. The complexity of this care situation highlights different logics, maneuvers and practices of the protagonists, giving meaning to the doctor-patient relationship. It is therefore necessary to analyze from the inside the context in which the patient meets the doctor. الملخص: تهدف دراستنا الى محاولة فهم وتفسير اهم العناصر المحددة لكيفية بناء اللقاء بين المريض المسن والطبيب في حالة المرض المزمن. تعتبر عملية بناء اللقاء بين هذين الفاعلين الاساسيين في الفضاء العلاجي، بعدا اساسيا في بناء العلاقة العلاجية، وذلك بالرغم من تجاهله لدى معظم الدراسات التي تناولت ميدان العملية العلاجية. فمن الضروري دراسة محددات وضعية اللقاء بين المريض والطبيب، اذ ان هاته الاخيرة تمكننا من الكشف عن طبيعة العلاقة التي تربط بين هذين الفاعلين، الى جانب العناصر المعقدة التي تتدخل في عملية التفاعل بين طرفي العلاقة. يمكن تعريف هذه العملية المعقدة والتي تبنى على اساسها العلاقة العلاجية، على انها تلك السلسلة من التفاعلات المتشابكة والتي تمكن المريض المسن من انتاج عملية تقييمية لنوعية الخدمات الطبية و"لكفاءة " الطبيب الممارس. لذلك أصبح من الضروري تحليل وتفسير السياق الذي يلتقي فيه المريض المسن بالطبيب.
