« Circuit Des Médicaments Radio Pharmaceutiques Dans L’unité De Médecine Nucléaire De L’ehs Pierre Et Marie Curie. »

Résumé

Les Médicaments radio pharmaceutiques (MRP) font l’objet d’un circuit du médicament particulier ; C’est pourquoi nous avons réalisé un audit qualité concernant la gestion des MRP au niveau de l’Unité de Médecine Nucléaire du service d’Endocrinologie (UMNSE) de l'EHS CPMC en s’appuyant sur les recommandations de l’AIEA, l'objectif principal était de vérifier les points critiques de gestion de la radio-pharmacie tels que : l'approvisionnement, le stockage, la reconstitution/distribution, la gestion des déchets ainsi que la sécurité du personnel et des patients. Aussi, la présente étude vise à améliorer et faire progresser les pratiques de gestion des MRP au sein l’UMNSE. En se basant sur les lignes directrices et les bonnes pratiques publiées par l'AIEA nous avons pu développer une liste de contrôle d'audit afin de faire un état des lieux comprenant la gestion de la radio-pharmacie, la radioprotection (du personnel /des patients) et la dispensation (99mTc/131*I). Les résultats de l’audit révèlent une relative bonne conformité de l’unité aux recommandations de l’AIEA; Néanmoins, quelques manquements sont à déplorer et ils ont trait principalement au manque de formation du personnel, l'absence de contrôle qualité des préparations réalisées et un suivi médical insuffisant du personnel. Par ailleurs, Nous avons également établi des questionnaires qui ciblent séparément les patients et le personnel afin d'évaluer leurs connaissances en radio-pharmacie et radioprotection. Aussi, après étude des résultats nous avons établis des recommandations et des documents informatifs ont été édités à destination des personnels et des patients. Les tests de connaissance relèvent des lacunes, notamment chez le personnel de catégorie B. Aussi, les patients se sont montrés intéressés et ont soutenu l'idée de leurs donner des explications orales et sur support écrit. En collaboration avec le service nous avons réalisé un dépliant attractif et facile à lire. Nous y avons mis des définitions importantes telles que la thérapie à l'iode et les instructions à destination des patients. Nous avons pour le personnel réalisé une affiche grand modèle qui fait des rappels en matière de radioprotection: matériel de protection, distance de sécurité, sensibilité des tissus aux rayonnements et suivi médical. -- Radiopharmaceuticals (RP) are subject to a specific drug circuit; This is why we carried out a quality audit concerning the management of RP at the Nuclear Medicine Unit of the Endocrinology Department (NMUED) of the SH CPMC based on the recommendations of the IAEA, the main objective was to verify the critical management points of radiopharmacy such as: supply, storage, reconstitution/distribution, waste management as well as the safety of personnel and patients. Also, this study aims to improve and advance RP management practices within the NMUED. Based on the guidelines and good practices published by the IAEA, we were able to develop an audit checklist in order to take stock of the situation including the management of the radio-pharmacy, radiation protection (of personnel / patients) and dispensation (99mTc/131*I). The results of the audit reveal a relatively good compliance of the unit with the recommendations of the IAEA. Nevertheless, some failings are to be deplored and they relate mainly to the lack of staff training, the absence of quality control of the preparations made and insufficient medical monitoring of the staff. In addition, we have also established questionnaires that target patients and staff separately in order to assess their knowledge of radio pharmacy and radiation protection. Also, after studying the results, we have established recommendations and informative documents have been published for staff and patients. The knowledge tests reveal shortcomings, particularly among category B staff. Also, the patients showed interest and supported the idea of giving them oral and written explanations. In collaboration with the service, we have produced an attractive and easy-to-read leaflet. We have included important definitions such as iodine therapy and patient instructions. We have produced a large model poster for the staff which provides reminders on radiation protection: protective equipment, safety distance, sensitivity of tissues to radiation and medical follow-up.
