Analgésie Péridurale Pour Le Travail Et L’accouchement : Connaissance, Acceptabilité Et Satisfaction Maternelle Chez Les Parturientes De L’ouest Algérien

Résumé

Abstract Introduction - The pain that accompanies childbirth is lived as a multidimensional experience. Its setting is absent in Oran hospitals. The present study aims to evaluate the knowledge and acceptability among parturients of epidural analgesia, and measure maternal satisfaction, after the establishment in our unit of an epidural analgesia protocol. Patients and Methods - This is a descriptive study where all parturients admitted for vaginal delivery were included. The variables studied were : age at cervical dilation on admission, visual analogue scale (VAS) scores, time to install analgesia, adverse effects related to the technique, duration of first phase and 2nd phase of childbirth, quality of analgesia knowledge acceptability and maternal satisfaction. Results - 120 parturients at term with cephalic presentation participated in the study. The mean duration of the first phase of labor (55.04 ± 36.20 min) was shorter in multiparous vsprimigest (p <0.0001) as well as second phase (p <0.0001). Primigests had a more painful work (p <0.05). 70% of parturients did not know the technique, while 30% knew that the epidural can relieve labor pain. The knowledge is more marked among primigestes. 11.7% of parturients had expressed the wish to give birth under epidural analgesia and 88.3% had accepted to benefit from the technique on the proposal of the anesthetist doctors. Demand was stronger among primigestes. The overall satisfaction of parturients was evaluated in the immediate postpartum. 96.7% of parturients were very satisfied with the management of pain during labor and delivery regardless of parity. Conclusion - Epidural analgesia is unknown to most parturients but accepted on the recommendation of doctors. Maternal satisfaction is largely determined by quality of care, pain relief, and absence of fetal-fetal complications.
